Nasal Surgery (Rhinoplasty) is performed for cosmetic alterations in the external shape of the nose and/or for breathing problems.
Extensive discussion with the patient is necessary to understand exactly what they want and what can be done to achieve these goals.
Rhinoplasty is a very individual operation. There is no "one size fits all" in rhinoplasty, however, the nose can then be surgically tailored for any given person. The surgery can be performed in the office or in the hospital operating room. No hospital stay is required. I usually sculpt the nose through a small incision on the skin between the nostrils. In some cases, it can be done through the nostrils themselves. The soft tissues and cartilage are then manipulated to change the external shape of the nose. In order to alter breathing, the cartilage between the nostrils in the central portion of the nose, the septum, is rearranged to allow the clear passage of air. Sometimes small glands in the back of the nose are removed in order to permit better airflow.
A dressing and a small metal splint will be applied and must be worn for approximately a week. The nostrils are packed with Vaseline gauze, which is removed in a day or two. Following the removal of the splint, the nose will be swollen for several weeks, first visibly, then more softly. (The facial skin, because of its rich blood supply, tends to swell significantly.) One can return to work as soon one feels able and the swelling subsides to a permissible level.
It depends on what is causing the breathing problem. Most of the time the breathing can be corrected separately so no external changes are made. Sometimes these two procedures must be done together. Therefore , if breathing better is one's only concern, then the external portion of the nose does not necessarily need to be altered.
As I mentioned earlier, rhinoplasty is a procedure oriented toward the individual. A very small nose may not look good on someone with a large or wide face. A better-proportioned nose may be a more reasonable alternative.
